The Consent Form will be emailed to you prior to your appointment.  Reviewing and signing it electronically prior to your appointment really helps check-in time to go smoothly, so we encourage you to do so.  If you do not have email or cannot access the form, we will be happy to review it with you when you arrive and you can sign it then.  Thank you!
